Discussion: Using the Walden Library

I am currently pursuing the family nurse practitioner program at Walden University. An area that interests me is endocrinology in the primary care setting. I feel that many patients are choosing to have their primary care team oversee their diabetes management as opposed to going to a specialist. It can become very stressful for patients to have to see many different doctors and in certain cases, it is completely necessary to see a specialist. Still, I do believe there are certain diseases that primary care should be able to treat such as Type 2 diabetes adequately. I began my search on Walden’s Library page. Walden’s Library page offers many different resources and databases. I started first by using CINAHL & Medline combined search, “CINAHL stands for Cumulative Index to Nursing & Allied Health Literature, and includes peer-reviewed articles, reports of evidence-based practice, case studies, systemic reviews, and more.” (Walden, n.d). Upon entering this site, I used two terms, endocrinology and primary care.

I came across a peer-reviewed article called An endocrinologist-supported intervention aimed at providers improve diabetes management in primary care site: improving primary care of African Americans with diabetes which concluded by explaining, “Feedback on performance aimed at overcoming clinical inertia and giving to internal medicine resident primary care providers improves glycemic control. Partnering generalists with diabetes specialists may be important to enhance diabetes management in other primary care settings.” (Phillips, et al, 2005). As a focus in my studies, I want to overcome clinical inertia and put ADA standards into practice, and feel comfortable with adjusting PO and insulin regimens for optimal glycemic control in the primary care setting.

Difficulties I experienced while searching for an article were upon typing in my search over 4,000 articles come up, I don’t think it is possible or realistic to be able to shift through every single article. It took me some time to choose an article that spoke to what I was researching. Many of the articles were abstract instead of being able to access full text.

I believe this database would be extremely useful for my colleagues as we are all in these graduate studies programs that require access to scholarly articles and evidence-based practice to apply that research in our practice. The online article Simplifying Synthesis states“Quality evidence-based practice initiatives and nursing care are based on understanding and evaluating the resources and research available, identifying gaps, and building a strong foundation for future work.” (Shellenbarger, 2016). Understanding and evaluating resources and research available is vital for professional growth so I would definitely recommend this database as I felt that it provides access to quality evidence-based practice while being easy to navigate.

The nursing profession is continuously evolving in its approach to patient care, and outcomes as members continue to undertake research, leading to evidence-based practice. For the scope of this discussion the peer-review article selected which relates to my current job as a dialysis nurse and interest is “Improving Patient Experience and Treatment Adherence in the Adult, Outpatient Hemodialysis Population” (Saunders et al., 2019). When searching for this article, the database used was CINAHL Plus with Full Text. I chose this database because it encompasses many nursing journals, standards of practice, and allied health evidence-based practice; therefore, I recommend its’ use. Also, the database holds familiarity since I used it during my undergraduate BSN program. There were no significant difficulties during the search for my article. I ensured that I did not use full sentences in the search fields but keywords related to my particular interest. To assess if a journal article was peer-reviewed, I utilize the periodical directory, Ulrich’s. This software does not contain stored articles but verifies if a journal uses peer-review. Besides using Ulrich’s software, I selected on the database the option to narrow search options to peer-reviewed journals only (Walden Library, 2020).

As a scholarly writer, the use of peer-review articles is an asset. Scholarly papers referenced with peer review articles provide expert verification of thoughts since experts review peer-review articles within their discipline (Glasgow Caledonian University, n.d.).

Peer-review research also forms an integral part in the development of the professional practice of nursing. History shows that nurses previously depended on individual experience, advice from senior nurses, and potentially outdated educational information, which led to varying patient outcomes. Due to the inconsistency in patient outcomes, physician David L. Sackett developed evidence-based practice. Patient care and treatment are based on research; current evidence, so optimal care can be provided. Therefore, since peer-reviewed research involves verifying information by established experts in the nursing field, information validity is increased (St. Thomas University, 2019).

This is insightful Gail-Ann. Peer reviewed research articles are important components of learning processes. Learning processes require conclusive research processes to make reference and to retrieve data that may be useful in making some conclusions. Scholarly journals and peer-reviewed articles have been written and proven by different professionals in different fields of study. These resources therefore act as rich sources of information and knowledge in various fields of study (Walden University Library, n.d).

Nurses need to engage in the research processes to ensure that they understand different aspects of healthcare system. Research processes and scholarly writings improves communication and clinical skills that are important for healthcare professionals. Also, with the current healthcare practices, there is always application of evidence-based practices which are important in ensuring effective patient outcomes. All these approaches require the knowledge of research processes during learning.

To find effective and relevant peer-reviewed research articles and other sources of information for the research processes, it is necessary to consider different databases (Thomas, Ciliska, Dobbins, & Micucci, 2018). Walden University has various databases containing millions of articles, journals, books, and other sources of information that can be integrated in the learning processes. Cumulative Index to Nursing & Allied Health Literature (CINAHL) is one of the databases used by Walden University students to retrieve different forms of information in the research process. In other words, the database consists of different peer-reviewed articles that have been published over the years by different professionals. These articles are on different topics and are related to nursing practices. Also, Medline integrates different research articles and journals that help learners in the research process. Medline integrate search from Walden University, as well as from other publications that are relevant to the nursing processes.

I searched the Walden Library for an article on closed glottis pushing during labor, also known as the Valsalva pushing. I am a labor and delivery nurse and I have experienced many different techniques, therefore closed glottis pushing in labor appears to be the most effective. It is based on high abdominal pressure induced by the descent of the diaphragm, which acts like a piston on the uterus and helps the mobilization of the descending fetus by compressing the uterine fundus (Barasinski, C., Vendittelli, F., 2016).

There were some difficulties for me to find the articles I needed being that I was not familiar with this system using this database. CINAHL is the research system I have used in the past for nursing and allied health professionals. A great future that I stumbled on was the Ask a Librarian. I reached out for text citations and reference formats. The time frame for a response was surprisingly quick.

Walden University Library has many options of course once you learn how to navigate through and enter all the correct keywords. Walden Library was challenging for me but well organized and provided me with valid information, other resources, peer-reviewed journals, conferences, papers, magazines, and other materials (Walden University Library, n.d.). Due to the ever-changing field of healthcare, it is important to regularly seek out new research articles. There are many different resources available to assist in obtaining such information that ensure that the articles provide initiative, quality, and evidence-based materials. So for all of the above reasons, I would say yes that I would recommend that the database would be very useful to my colleagues.

Until recently I did not know about the specialty of psychiatry mental health nurse practitioner (PMHNP), and or advance practice registered nurse (APRN). I knew of family nurse practitioner (FNP). Although it may sound a bit strange, we are from different parts of the world doing the same course and providing the same nursing care. According to Kaye et al. (2009) child and adolescent mental health disorders is likely to be exacerbated over time due to a shortage in age-appropriate mental health services. Psychiatry mental health nurse practitioners bridge the gap in prescribing and/or monitoring psychotropic medication when child and adolescent psychiatrist (CAP), are unavailable. Been in a specialty that is in high demand that uses evidence-base nursing practice are driving the force in our society. 

I choose to CINAHL plus with full text database to search for a peer reviewed article to support this topic, not because I knew about the database, but because it was the first database at the top of the page. I type in a few key words and got a substantial amount of articles. I further narrowed my search by clicking the peer reviewed option which provided, article, journals and books with evidence that supports my thoughts. It was extremely user friendly, you can limit your search to publication date such as year, month, geographic subset, age, sexand many more options. I plan on continuing using these resources provided through Walden Library for my scholarly writing and recommend my colleagues and classmates to try it as well as all the other databases.
