Over the last eight weeks we have been discussing how important evidence based research to the nursing field. Evidence based research lays the foundation for evidence based practice. Evidence based practice aids in the development of policies and procedures in all healthcare organizations. When I was in nursing school I remember how much instructors embedded the importance of evidence based practice in to our heads. As Lisa Spruce (2015) discusses patient outcomes and core measures improve when healthcare providers follow evidence based practice (p. 110).

As I learned in previous classes part of being a good leader is to guide others. If I plan to develop an environment that fosters evidence based practice I need to lead by example. There are numerous ways I can complete this. I will begin with incorporating evidence based practice in to my work. I will discuss evidence based practice when interacting with colleagues. When I am asked questions I will encourage the utilization of evidence based research to answer the questions. In addition, I refer to evidence based research when answering questions. When teaching colleagues a skill set that is new to them I will refer to evidence based research that support why we complete a task the way we do.

In addition to role modeling evidence based practice in to my everyday work I will educate the organization as a whole to promote evidence based practice in to the organization’s culture. I would hold educational classes to teach the benefits of evidence based practice. All members of the organization would be required to attend these classes. The classes will need to be tailored to the audience I am teaching. According to Spruce (2015) strategies to implement evidence based practice include interactive education and the use of evidence based practice publications (p. 109-110). In addition I will encourage testing new evidence based practice recommendations in certain areas of the hospital. This is currently happening at the organization I currently work and it is really neat to see how it unfolds.

As we know there is always resistance to change in the beginning. I cannot count how many times I have heard “this is how we have always done it”. With all the advances in healthcare change is in enviable. In order to change the mind of someone who is resisting the change I need to be creative. I would begin with the basics. This would include leading by example and referring to evidence based research. I would also show how the changes benefit the patients. In order to do this I would refer to any pilots we are doing and present the improvement in patient care these changes are making. I would also encourage them to be part of team that is piloting a change so they can see firsthand how this will benefit the future of healthcare as well as our patients.

This weeks discussion looks forwards and asks about the future use of evidence-based practice in our workplace setting.  We are to imagine ourselves as nursing leaders in the advance practice setting and answer the listed questions above.  To begin, i would role model evidence-based practice in my future setting by basing decisions and practices on the latest research.  This can be achieved by using transparency with co-workers as to why a certain change was implemented and needed.  At my current place of employment, certain protocols such as sepsis bundles were rolled out to staff in this manner.  The need for a change to old protocols was first identified using current data at the time that showed a delay in our implementation of sepsis treatments as well as data showing our patient outcomes compared to others.  Then, the latest research was shown to staff showing how effective new sepsis bundles were for patients and this was used as the reason for needed changes.  Committee’s were also formed so that any staff member that wanted to participate in the research and changes could do so.  The successful implementation of EBP is a dynamic process dependent on a number of variables. Individual experiences, bias and attitudes alongside professional, organisational and workplace factors can act as hurdles or barriers to the translation of empirical knowledge into practice and as such this process can take many years. (Williams, Perillo, & Brown, 2015).  In order to foster a culture of EBP for an organization, it takes time, competence and motivation.  Members of the organization from top to bottom must lead by example and show that they also value this type of culture.  An open-door, non-punitive  policy to new ideas for needed changes is a great method to promote this.  Asking staff to openly search for areas of improvements and forming committee’s to research how to resolve problems is another method that allow staff to become part of the process for researching and implementing changes.  If a staff member is stating that things have never been done this way, they may be resistant to change.  I would offer them to participate in a committee or current project as well as educate them on the importance of EBP in the workplace.

Evidence-based practice (EBP) plays an important role in healthcare.  The goal of healthcare providers are to improve the populations’ health status and reduce health disparities.  It is the driving force for many quality improvement initiatives and the standardization of healthcare practices and polices (Brower, 2017).  In order to role model for EBP in my future setting as a family nurse practitioner, I would first explain EBP and its importance in the clinical setting.  I would identify barriers of EBP implementation and promote strategies to eliminate the found barriers.  Many healthcare providers are not clear on the meaning of EBP on a professional level and personally applicable level, and face barriers when attempting implementation (Brower, 2017).  Lastly, a creation of an environment that promotes learning and development of EBP along with training programs that details EBP and its use in the clinical setting. 

I would foster organizational culture that promotes EBP by organizing EBP activities.  My behaviors as a role model will help others learn about EBP and how to achieve the results.  The use of role modeling will assist the organization with becoming aware of the need for EBP and gain the knowledge and skills to promote EBP within the organization.  In order to promote EBP practice throughout the entire organization, I must ensure that managerial staff is aligned with the use of research and EBP activities.  The use of in-services for all staff will help facilitate knowledge and skills that can be utilized throughout the organization.

As an advance practice nurse, in order to change the staff member from distracter to a promoter of EBP, I must allow opportunities for the staff member to engage in EBP work on a daily basis.  This will allow the distracted staff member to gain increase knowledge, while promoting a sense of normalcy.  The advance practice nurse should support the staff member by continuing to participate in daily evidence-based practices as well.  The leadership and role model behaviors by the advance nurse will mirror the actions of evidence-based practices that are needed to advance the entire organization in the improvement of better patient outcomes, which is the main goal of evidence-based practices.
